In accordance with that defined by Law 1581 of 2012, Regulatory Decree 1377 of 2013, External Circular 002 of 2015 issued by the Superintendence of Industry and Commerce and other concordant rules, through which general provisions on habeas data are established and the processing of information containing personal data is regulated, I hereby expressly declare that:

• I freely and voluntarily give my personal data to Bits de Bienestar S.A.S., in that sense, I declare to know that the personal data object of treatment, will be used specifically for the purposes derived from the corporate purpose of Bits de Bienestar S.A.S.
• I authorize in a free, voluntary, previous, explicit, informed and unequivocal way to Bits de Bienestar S.A.S, so that in the legally established terms, to carry out the collection, storage, use, circulation, suppression and in general, the treatment of the personal data that I have proceeded to deliver or that I will deliver, by virtue of the legal, contractual, commercial relations and/or of any other that arises, in development and execution of the purposes described in the present document.
• Such authorization to carry out the processing of my personal data, extends for the entire time in which a link may be consolidated or persists for any reason with Bits de Bienestar S.A.Sy after the termination of the same, provided that such treatment is related to the purposes for which the personal data were initially provided.
• Likewise, I declare that I have been informed and I know the rights that the legal system and jurisprudence grant to the holder of personal data and that include among other prerogatives those listed below: (i) To know, update and rectify personal data against those responsible or in charge of the treatment. This right may be exercised, among others, against partial, inaccurate, incomplete, fractioned, misleading data, or data whose processing is expressly prohibited or has not been authorized; (ii) request proof of the authorization granted to the data controller, except when expressly exempted as a requirement for the processing; (iii) be informed by the data controller or data processor, upon request, regarding the use made of my personal data; (iv) file complaints before the Superintendence of Industry and Commerce for infringements to the personal data protection regime; (v) revoke the authorization and/or request the deletion of the personal data when the processing does not respect the constitutional and legal principles, rights and guarantees; (vi) access free of charge to my personal data that have been subject to processing.
